Clinical Social Worker

It is preferred that the social worker have at least one year of experience in a healthcare setting.

All social workers must be able to comply with licensure laws in the states in which he/she is assigned.

 The social worker must:

• have a Master of Social Work (MSW) from a school of social work accredited by the Council on Social Work Education; or
• have a baccalaureate degree in social work (BSW) from an institution accredited by the Council on Social Work Education; or
• have a baccalaureate degree in psychology, sociology or other field related to social work
• have the ability to effectively communicate verbally and in writing, with clients and other staff members

• have the ability to assess the psychosocial needs and coping ability of the Hospice patient/family unit in a variety of settings and with varied lifestyles

• be able to participate in the development of the Interdisciplinary Team’s plan of care and provide social services within that plan of care

• be able to document accurate reports of assessments and care provided

• be able to maintain contact with third party insurance carriers regarding the family’s need for services and inform the family and staff of specific insurance requirements for hospice care coverage

• be able to help families problem solve
• be able to load and unload medical equipment weighing up to 45 pounds

• be physically able to visit clients in their homes
• be able to drive in all types of weather

• have his/her own reliable transportation and a valid driver’s license meet regulatory personnel file requirements and be able to meet the Hospice of Dubuque Code of Organizational Ethics and Standards of Conduct.                                                                              

 Although hospice care is a specialty, Hospice of Dubuque will provide the orientation, mentoring, and ongoing education necessary for the Hospice Social Worker to be competent in the care he/she will be providing.
