Your Gift Can Help!

           Hospice of Dubuque is grateful for your support. Your generosity allows us to continue to offer our            compassionate care to those with terminal illnesses and their families regardless of income, insurance            coverage or inability to pay for the care. Our bereavement and educational programs are supported,
           in their entirety, by fundraisers, donations and memorials.

Donations may be sent to Hospice of Dubuque by:

• making checks payable to Hospice of Dubuque or supplying your Visa/Mastercard
  information;
• including a note indicating who the gift is in honor or memory of;
• including the name and address of the family member you wish us to notify of your donation (the amount is never mentioned);
• mailing a gift to Hospice of Dubuque, 2255 JFK Rd., Dubuque, IA 52002;
• or by calling our office at 563.582.1220.

We will promptly acknowledge your gift with a receipt. If requested, we will also send notification of your gift to the person(s) you have identified.

Matching Gifts

Your donation may go further than you thought. Thousands of major corporations match employee charitable contributions. Check with your employer to determine if Hospice of Dubuque is eligible for a matching gift and to complete the necessary documentation.

Hospice of Dubuque is a 501(c)(3) nonprofit organization and donations and grants are tax deductible to the fullest extent allowed by the law.
