• the patient's physician
• the medical director
• registered nurses
• social workers
• a chaplain
• therapists
• a registered dietitian
• a pharmacist
• home health aides
• a volunteer coordinator
• and specially educated community volunteers
This care team works together with the patient and their family to make the individual's remaining months and weeks of life as meaningful and comfortable as possible.

The Hospice Nurse must:
• be licensed as a Registered Nurse, and/or willing to obtain appropriate licenses to
provide care in the Hospice of Dubuque service area
• have the ability to effectively assess and respond to the needs of patients and families in
a variety of settings and with varied lifestyles
• be able to communicate effectively with clients and other staff members
• be able to participate in the development of the Interdisciplinary Team’s plan of care,
and provide nursing care within that plan of care
• be able to document accurate reports of assessments and care provided
• participate in on-call responsibilities
• be able to load and unload medical equipment weighing up to 45 pounds
• be physically able to visit patients in their home, and provide full physical care to bed-
bound patients in low beds
• be able to drive in all types of weather
• have his/her own reliable transportation
• meet regulatory personnel file requirements
• be able to meet the Hospice of Dubuque Code of Organizational Ethics and Standards of Conduct
Although hospice care is a specialty, Hospice of Dubuque will provide the orientation, mentoring, and ongoing education necessary for the Hospice Nurse to be competent in the care he/she will be providing.

There is a competitive wage and benefit package that recognizes employee excellence. Although the position is client-driven, part to full-time hours are generally Monday through Friday during the day. Occasional evening or week-end hours may be needed.
The Hospice Social Worker must:
• be a BSW or BA graduate of a school accredited by the Council of Social Work Education with at least one year of social work experience and the ability to maintain current licenses as required by the states in which duties are performed. Prefer a BSW with a MSW and experience in the healthcare field. Related degrees cannot be considered.
• have the ability to visit clients in their homes and effectively assess the psychosocial needs and coping ability of clients in a variety of settings and with varied lifestyles. The position includes providing bereavement care to families after the death of a loved one.
• be able to assist client families with financial concerns, linking the family to appropriate community resources for financial or other social service needs and to act as a client advocate in the community.
• be able to provide client families with information regarding their rights, responsibilities and advance directives, and to participate in volunteer and community education.
• be able to participate in the development of the Interdisciplinary Team’s plan of care, and
provide social services and emotional support within that plan of care in a timely manner.
• be able to assist families needing hired caregiver arrangements or alternate placement when home care is not possible.
• maintain contact with third party insurance carriers regarding the family’s need for services, informing family and staff of specific insurance requirements for hospice coverage.
• be able to provide psychosocial and client problem-solving, and consultation to staff members as needed.
• be able to communicate verbally, and in writing, with clients and other staff and be able to document client assessments, interventions and outcomes in a timely fashion.
• have own reliable transportation, be able to drive in all types of weather, and be able to load and unload medical equipment weighing up to 45 pounds
• meet regulatory personnel file requirements and abide by Hospice of Dubuque policies and procedures as well as all applicable laws and regulations.
• be able to meet the Standards of Conduct and Hospice of Dubuque Organizational Code of Ethics, including maintenance of strict confidentiality of all agency and client information.
Although hospice care is a specialty, Hospice of Dubuque will provide the orientation, mentoring, and ongoing education necessary for the Hospice Social Worker to be competent in the care he/she will be providing.
