Services

The Hospice of Dubuque Interdisciplinary Team combines the skills and compassion of varied professionals and volunteers to enhance end of life care.  Included as a part of this care team are the patient, family caregivers and the patient’s attending physician.  Care is individualized to each patient and family, thus enabling them to have their preferences and goals honored.  The Hospice of Dubuque care team members provide expert medical care, pain management, emotional support and spiritual guidance.  Services are focused upon promoting family control, patient comfort and quality and dignity of life at the end of life.

Physicians

Hospice of Dubuque works with the patient’s primary physician. It is not necessary for the patient to change their physician in order to utilize hospice services. Hospice of Dubuque also has medical directors who oversee the medical component of the program. The medical directors attend weekly team meetings to offer additional medical direction for the patient’s care.

Nurses

Registered nurses are available to address physical needs of the patient as well as the emotional needs of both the patient and their family. Upon admission, the patient is assigned a primary nurse who will visit on a regularly scheduled basis. The frequency of these visits will vary according to the patient’s need. Between these visits, an on-call nurse will be available 24-hours a day, 7 days a week.

Hospice of Dubuque also relies on experienced, part-time team nurses who work in conjunction with the hospice nurses to complete assessments and provide direct care to the Hospice of Dubuque families.

 

Social Workers

A social worker is assigned to each family upon admission and is available to meet with the patient and family on a regular basis to address any emotional or financial needs they may have. The social worker can arrange for volunteer support and home health aide services. The social worker can also provide information on community resources.

Home Health Aides

Hospice of Dubuque home health aides are available on a scheduled basis if the patient desires assistance with personal cares, such as bathing, shampoo or nail care.

Dietitian

The role of the dietitian is to be a resource regarding any food, appetite or nutrition related concerns for the dying patient. This could range from changes in eating patterns and food textures to questions concerning tube feedings and allowing the patient to refuse food as a natural course of the dying process.

Pharmacist

An active participant in patient care, the pharmacist, serves as a source for current drug information and as a consultant for changing drug regimens. The pharmacist reviews the patient and family’s plan of care and offers medication related guidance to the health care team.  The pharmacist is guided by repeated assessments, with adjustments in medications, and times of administration so that the patient will be as comfortable as possible.

Therapists

The music therapist is available throughout the patient's hospice experience to provide music-based interventions to help patients relax, improve comfort, elevate mood and to reach non-verbal patients.

Other professional therapists, such as physical, occupational and speech therapists may assist with special needs as assessed by the hospice team.

 

Spiritual Care

The spiritual care coordinator works with the patient, family and their clergy to ensure that all spiritual needs are being met. If desired, the spiritual care coordinator can provide direct spiritual support with or without an affiliation with a local church.

Bereavement Care

Bereavement services are extended to all Hospice families following a death.  The bereavement coordinator will explain these services, giving families the option of utilizing any or all of them.

Volunteers

A volunteer coordinator will contact the family shortly after admission to explore the need for volunteer support. Volunteers complete an intensive hospice educational program and can provide a variety of services for a limited number of hours each week. The volunteer coordinator will explain how the volunteers can be helpful to the family.

Community Education

Hospice of Dubuque has a responsibility to our community to provide education to individuals, groups and organizations. We are willing to provide educational presentations on hospice care, history and services. Presentations are offered at no cost and can be scheduled at your convenience.
